CONTENT CREATOR EXPERIENCE DISCLOSURE

By booking an appointment, participant acknowledges and agrees that the services provided are part of a filmed content creation, performance, modeling, entertainment, and media production experience.

All participants must execute any required model release, consent forms, and participation agreements before any filming, photography, recording, or content creation activities begin. Failure or refusal to complete required documentation will result in cancellation of the session.

These experiences are intended for the creation of digital media, artistic expression, entertainment content, and performance-based productions. Any physical interaction is incidental to the performance and content creation process.

Participant understands that no medical, therapeutic, chiropractic, physical therapy, rehabilitation, healthcare, or professional massage services are being offered or provided through these content creation experiences. No diagnosis, treatment, or claim of health benefit is made.

Participant acknowledges that participation is voluntary and that they may withdraw consent to continue participation at any time prior to the commencement of filming or recording activities, subject to any applicable booking policies.

Any fees paid are compensation for the creator's time, appearance, production efforts, scheduling, travel, studio expenses, administrative costs, and participation in the content creation experience.

All participants affirm that they are adults at least 18 years of age and legally capable of entering into binding agreements.

By proceeding with a booking, participant acknowledges that they have read, understood, and voluntarily agree to this disclosure and all associated participation documents.
